Vital Records - Marriage Birth & Death Records for Keokuk County, Iowa
Birth Records
Birth Records are available for years 1880-1920 and 1942 to current for customers applying for mail. Birth Records from 1921 to 1941 are available only to walk-in customers in the office due to state regulations and are available by mail through the Iowa Department of Public Health. Births to unwed mothers prior to 7/1/1995 are only available at the Iowa Department of Public Health. See below for certified copy requirements.*
Death Records
Death Records are available for years 1880-1920 and 1942 to current for customers applying for mail. Death Records from 1921 to 1941 are available only to walk-in customers in the office due to state regulations and are available by mail through the Iowa Department of Public Health. See below for certified copy requirements.*
Marriage Records Marriage Records are available for years 1854 to current. See below for certified copy requirements.*
*Certified Copy Application Instructions:
Certified copies are available to only those with entitlement. (i.e. person named on record (if 18 or older), mother or father (if named on record), spouse, brother or sister (if 18 or older), maternal grandparents and paternal grandparents. (if father is named on record), children or legal representatives with entitlement of proof of blood relation or representation may be required.
To receive a certified copy of a Keokuk County Vital Record:
1) Complete Application for Certified Copy form which is required to be signed in front of a notary public; and 2) Provide copy of a current Driver’s License or other current government photo ID; and 3) Submit $15.00 check or money order to:

County vital record information is available by mail only in the form of a certified copy through a completed mailed application, proof of a government issued ID and receipt of payment by individuals entitled to the record.
Marriage Application and Licenses:
To apply for a marriage license, the bride, groom and one witness (age 18 or older who knows both parties) must sign an application before a notary. Applicants & witnesses are required to show valid photo ID's, such as a drivers license. Parental consent required for marriage license applicants age 16 and 17. Marriage application fee is $35.00. Application requires a three working day waiting period and is valid anywhere in the state of Iowa. See state marriage license instructions.
